Initial Screening Service
Before we can process your referral, we need to check that we are the right service for you through our Initial Screening Service.
What Does This Mean?
We will ask you to complete various forms regarding details of you, and/or your child/young person, as well as strengths, challenges and background information.
Once completed, the clinical team will meet as part of a Multi-Disciplinary Team (MDT) meeting to discuss the referral information. They will decide whether we, as a service are able to accept your referral at this time. Please refer to our Inclusion and Exclusion Criteria to review whether your child/young person, or yourself, are suitable for our services.
The MDT will communicate the outcome in a letter. This letter will contain the clinical rationale for our recommendation.
Please note the initial screening stage is non-refundable; therefore, please ensure you have reviewed our Inclusion and Exclusion Criteria and are happy to proceed to the next stage. If you have any questions, please before completing the purchase.
The completion of this screening form does not guarantee registration with Young Wellbeing Hub.
All submissions are subject to review by our clinical team. Following review, you will receive a letter confirming whether we are the most appropriate service for you, the clinical rationale for this and relevant recommendations.

Once the Initial Screening has been completed you will either progress to our Psychological Consultation with one of our YWH clinicians, or access our diagnostic and psychiatric services.
Fees
The fee for our Pre-Screening Service is £149. The fee for our Pre-Screening Service and Psychological Consultation in total is £250.
If your referral is not accepted after pre-screening has taken place, and we are unable to progress you through to the Psychological Consultation, you will be refunded a total of £101 (the cost of the consultation).
